Quiltmaker's Documentation Form

I just added a free Quiltmaker’s Documentation form to the informational page called “Documenting Your Quilt” which is under the “Protecting Quilts” tab above. If you are a quiltmaker, I encourage you to take the time to document your quilts. Your written documentation becomes so important if your quilt becomes missing, but it is also valuable for your family. As someone who has a father with Alzheimer’s Disease, the things he took time to write down have become such a treasure.

Please feel free to make copies of this quilt documentation form and distribute it to your friends, your quilt guild, or wherever you like. May your quilt never be among the missing!

    Thanks for the form, I know I should be documenting when I make quilts….but now I have a form to fill out already for me. I think I’ll add a picture and then put them together in a binder.

  2. Hope it will be helpful! I just finished making a few changes (2:30 PM EST on 1/16/11), so if you downloaded an earlier version, you might want to get the newest one :)
